Information about what to expect and what you’ll need when you come to see Dr. Frank for the first time.
First Visit – Financial Information
For your convenience, we accept Visa, MasterCard and American Express. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us. Many times, a simple telephone call will clear up any misunderstanding. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. Third party payment programs such as Care Credit and Citi Health Card are available.
First Visit – Consultation
A consultation appointment will include a review of your medical history, an examination and an explanation of your diagnosis and treatment options. Surgery can sometimes be performed the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment.
Please bring with you at the time of your consultation:
The medical information form that may have been sent to you
Your surgical referral slip and any X-rays if applicable
A list of medications you are presently taking
Any medical or dental insurance information and /or ID cards. We can not help you process any claims without these.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit to discuss and obtain consent. A pre-operative evaluation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink 6 hours prior to your surgery. You will also need an adult to drive you home or we can arrange appropriate transportation for you.
Also, please wear loose fitting clothing if possible. Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. uncontrolled diabetes or high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.). Anticoagulants are medications that can thin your blood, resulting in increased bleeding (aspirin, coumadin, or Plavix).
First Visit – X-Rays
X-Rays are usually necessary to diagnose problems of the teeth or bones. Your dentist or physician my have taken recent ones which we may be able to use for diagnosis or treatment. You can make arrangements to have them sent. If the images are not diagnostic for our purposes or they are not available at the time of your visit additional radiographs may be necessary.
First Visit – Scheduling
The office is open Monday, Tuesday, Thursday and Friday from 9 am until 4:30 pm. We will schedule your appointment as promptly as possible. If you have pain or an emergency situation every attempt will be made to see you as soon as possible.
We value your time and try our best to stay on schedule to minimize your waiting. However, because we provide surgical services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.
Please call with any questions or to schedule an appointment. Please advise us if you have undue anxiety or concern about your appointment. We will make every effort to minimize your fears and answer your questions.

First Visit – Insurance Information
We participate in many but not all dental plans. We are happy to file your insurance forms if you provide accurate information. If you have in-network benefits it is best to confirm with your plan administrator or insurance company that they apply in this office. If you like we can request written pre-treatment estimates for dental procedures from your insurance company for non-emergent treatments.
First Visit – Payment Plan
We participate with third party financing plans offered by Care Credit http://www.carecredit.com and Citi Health Card http://www.healthcard.citicards.com. Please let us know if you would be interested and we’ll help to make the arrangements which can be done in minutes.
First Visit – Privacy Policy
This office complies with federal HIPAA statutes protecting your health information privacy. You will be asked to read and sign the privacy notice. You can receive a copy of the form or refuse to sign it. We must document that we attempted to offer it. Please list any family members or others that you would like us to be able to share your health information with.
